Clinical application
《Guidelines for the Use of Laboratory Testing in the Diagnosis and Management of Thyroid Disorders》
Clinical Significance of rT3:
Hyperthyroidism: Serum rT3 is typically elevated, paralleling increases in T4 and T3. In some cases, elevated rT3 may be the earliest detectable abnormality in initial or recurrent hyperthyroidism.
Hypothyroidism: Serum rT3 is decreased. Measurement of rT3 aids in distinguishing primary
Hypothyroidism from the euthyroid sick syndrome (non-thyroidal illness syndrome, NTIS). NTIS: Elevated serum rT3 with a reduced T3/rT3 ratio is characteristic of systemic illnesses such as acute myocardial infarction, liver cirrhosis, diabetes mellitus, uremia, and critical illness. This pattern serves as a valuable marker for assessing disease severity, monitoring therapeutic response, and predicting prognosis.
《Guidelines for diagnosis and management of hyperthyroidism and other causes of thyrotoxicosis in China》
Recommendation 3-3: The third-generation TRAb assay can be used for the etiological differentiation of thyrotoxicosis and the diagnosis of Graves' disease hyperthyroidism. (Strong recommendation)
《Expert Consensus on Diagnosis and Treatment for Elderly with Thyroid Diseases in China(2021)》
Thyroid‐stimulating hormone receptor antibody (TRAb) is a crucial marker of Graves' disease. TRAb is positive in approximately 60%–90% of patients with first‐onset Graves' disease. TRAb can also be positive in patients with Graves' ophthalmopathy with normal thyroid function. In addition, TRAb has certain significance in predicting recurrence of hyperthyroidism after withdrawal of antithyroid drug therapy
