HLH (Hemophagocytic Lymphohistiocytosis), also known as Hemophagocytic Syndrome (HPS), is a severe inflammatory reaction syndrome caused by genetic or acquired immune dysregulation. HLH is a rapidly progressive and highly fatal disease, with a median survival time of untreated HLH not exceeding 2 months. HLH lacks specific clinical manifestations and is prone to misdiagnosis and missed diagnosis^[1].

IL-2R One of the diagnostic indicators recommended by domestic and international guidelines/consensus

International Guideline Article 8 of the HLH-2004 diagnostic criteria: elevated sCD25 (soluble interleukin-2 receptor).

International Guideline Article 4 of the diagnostic criteria in the Chinese Guidelines for the Diagnosis and Treatment of Hemophagocytic Syndrome (2022 Edition): sCD25 (IL-2R) level ≥6400pg/mL can be used as one of the diagnostic indicators.

Efficacy Monitoring of CAR-T Cell Therapy^[2]

CAR-T (Chimeric Antigen Receptor T-cell) is a major breakthrough in tumor immunotherapy, showing excellent efficacy in the treatment of hematological malignancies such as acute lymphoblastic leukemia, non-Hodgkin's lymphoma and multiple myeloma.

CRS (Cytokine Release Syndrome) is the most frequent and prominent acute adverse reaction in CAR-T therapy, with an incidence of 30-95% in lymphoma cases.

Baseline detection indicators: CRP, IL-6, IL-1, IL-2, TNF-a, IFN-γ, IFN-a, etc.

Laboratory test frequency: Before CAR-T cell infusion, on day 3 (d3), d7, d10, d14, d21, d28 after infusion, and at 3, 6, 9, 12 months after infusion; high-risk cases should be tested every 2-3 days after cell infusion.

Early Prevention of Sepsis

Sepsis is a life-threatening organ dysfunction syndrome caused by a dysregulated host response to infection, with a high mortality rate. The incidence of sepsis in ICUs in China is 20.6%, with a mortality rate as high as 36.6%^[3].
